INSTRUCTIONS

1. Please Fill up the form with all the details whatever you have.
2. Scan all the documents and your photo mentioned in the form. Keep it ready for uploading. It should be in JPEG format.
3. For Demand Draft(DD):Scan your DD and upload it, It should be in JPEG format.(Ignore if you pay online)
4. For Online Payment:Keep your debit card or credit card ready to fill up the number of your card.

5. Keep your password ready. As per Reserve Bank’s guidelines, you must enter your password if you are using the card for the first time. If you don’t have the password, you can get the password online by clicking “ GET PASSWORD ” from that page
6. If it shows “ time out ” you have to re-do the whole process again. To avoid that situation, keep all documents ready including your password.
